On Jan 17, 2011 around 6:30 PM, near 3500 Piedmont RD NE, Atlanta, GA 30305, driver (孩子名字）(driving ....)accidentally damaged Dxx's car.
Attached please find a check of $768.63 (Check #1002, Wells Fargo Bank) payable to Hxxx Dxxx. Upon cashing this check, Hxxx Dxxx. agrees to release (孩子名字）from all liabilities and waive all claims, suits, and causes of action arising directly or indirectly in connection with this incident, including but not limited to costs of car repair, bodily injury, mental suffering, time and income loss, and car depreciation value. Furthermore, Hxxx Dxxx declares that she has not engaged into any actions, claims, demands, causes of action or suits, including but not limited to claims and suits for bodily injuries related to the aforementioned accident preceding the execution of this Release of Accident Claims.
