Want to convert my own company type from INC to LLC. This requires the form 14-11-212, 14--11-204, and BSR231. I got the form BSR231, but there is no form 14-11-212 and 14--11-204 from the gov web to download. Any body know how to apply the conversion? Or may need a lawer for it? Any lawer I can consult?
